Coil Clinic – important information for patients

If you have booked an appointment at our coil clinic, please make sure you complete the following steps as soon as possible:

• Complete the pre‑assessment questionnaire
  This must be done at least 4 weeks before your appointment, otherwise it may need to be cancelled and rebooked.
  Complete the questionnaire here
• Collect a self‑swab kit and urine bottle
  These are available from reception.
• On the day of your appointment
  Please bring the first urine sample of the day with you, which we will use for a pregnancy test.
