Practice Nurse Trainee

We are looking for a forward thinking, conscientious and compassionate Nurse to join our nursing team. You will be registered with NMC and may be newly qualified or already have experience of working within a GP Practice or PCN setting. There is the opportunity to develop within the role and learn new skills.
Position: Practice Nurse Trainee
Location: Silverdale Practice, Burgess Hill, West Sussex (part of Burgess Hill Villages PCN)
Hours: 18-24 hours per week
Pay rate: £29000 to £35000 pro rata depending on experience
Scope & main responsibilities:
- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice. To develop, implement and embed health promotion and wellbeing programmes
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
- To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health
- As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
- Work within own competence and professional code of conduct as regulated by the NMC
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
- Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
- To be able to perform all practice nurse responsibilities such as childhood immunisations and cytology
Requirements:
NMC Registration: Valid registration with the Nursing and Midwifery Council (NMC).
Excellent Communication Skills: Ability to communicate effectively with patients, families, and colleagues in a supportive and timely fashion.
Empathy and Compassion: Demonstrated commitment to patient-centred care.

We have a team of 5 Partners, 6 associate GPs, a paramedic practitioner, clinical pharmacist, care coordinators, social prescribers, an experienced nurse team including 3 nurses and 3 HCAs and a very experienced and compassionate reception team. We have an experienced senior management team, led by our Practice Manager and we use EMIS clinical system, AccuRx and eConsults.
