Accessing your GP-held records via the NHS app or NHS website

Your GP medical record includes sensitive information such as consultation notes, prescriptions, test results, allergies, vaccinations, medical conditions, and documents from hospitals or other agencies.

We support giving you access to your record, but it’s important to ensure this is safe and appropriate. If you’re concerned about privacy – for example, if you share devices or are in a pressured relationship – we recommend speaking to us first. Online access is optional, and you can choose to opt out or request partial access (appointments and prescriptions only), which will be granted automatically.

Full access will only be enabled after a review of your notes. If we believe it may cause harm or distress, we will contact you to discuss further.

Please read the leaflet above to fully understand what access involves and how to proceed.
